Two human infections with Borrelia miyamotoi were identified in Slovenia in adults with undifferentiated fever. Both patients reported recent tick bites, and the illness was mild and self-limiting. These findings highlight the need to consider this pathogen in the differential diagnosis of fever following tick bites.
B. miyamotoi was detected in 2 of 337 adults by metagenomic sequencing and PCR. The first case was a 36-year-old man in July 2022 with fever, headache, and abnormal laboratory findings. The second case was a 31-year-old woman in May 2025 with fever, chills, nausea, and headache, as well as paresthesia. Both patients recovered without neurological complications or requiring antimicrobial treatment.
